Service Agreement
You will be asked to provide basic medical history on a written form prior to your session. All information you provided is protected by your patient rights, to be shared only with your consent. Please wait in the lobby until called into the room as there may be a session in progress. In the advent of inclement weather, please call and your appointment can be rescheduled when conditions are safe. There is a 24 hour cancellation policy in effect. By the courtesy of calling in advance you may reschedule your appointment. If you are late, there is a risk losing that time from your session however all is done to keep the full time. There is a $25 charge for a missed appointment.
